 Sports Qualifying Physical Information

Physicals are good for 3 years (36 months) from date of your last physical. Athletes must have a current sports qualifying physical exam on file in the Activities Office. 

Once you have your physical completed, you can turn it in to the office or you can scan it and submit completed Physical Forms online.
